Olyphant Ambulance offers a Yearly Membership Plan for the residents of Olyphant Borough.
Membership provides coverage for the following Emergencies:
1. Care and Transport of sick or injured patients to an appropriate medical facility in Lackawanna County, 24 hour per day - 7 days per week.
2. Transfer of patients from one area hospital to another when medically needed.
Membership Rates:
1. Single Person (1 patient) = $30.00
2. Family (2 or more patients) = $40.00
Membership Timeline:
May 1, 20xx through April 30, 20xx
Membership Provisions:
With Membership you are authorizing Olyphant Ambulance to bill and receive payments from your insurance carrier.
Any balance not covered by your insurance carrier will be covered by your Membership Plan.
Olyphant Ambulance bills at a base rate of $350.00 plus $8.00 per loaded mile.
Olyphant Ambulance does not bill for Paramedic Assists and is not responsible for additional charges.
